Ordering Instructions

Order Form

Please print out order form and follow these instructions.

  1. Check off color desired.
  2. Write the words 7" plate, 10" plate, 11" plate, 12" platter or accordingly under Description.
  3. Write the number of the design wanted. (Choose from colored examples.)
  4. Write the number of the border wanted. (Choose from colored examples.)
  5. Write the personalized information for the back of the plate. Example: Love, Bob & Deb, or, Merry Christmas, etc.
  6. Write the price of the item. Add 6% Sales Tax (PA residents only). Add the postage and shipping charges. Total the column.
  7. Check the appropriate box. If paying by MasterCard or VISA, Am. Express and Discover, please fill in account number and expiration date.
  8. ALL CUSTOMERS SIGN THE ORDER, verifying it.

DELIVERY TIME:

  • January thru May; 4-5 weeks
  • June thru December; 7-8 weeks
  • Rush Orders; 2-1/2 weeks ($10 extra)

*All our products are hand sculpted, painted and fired. Slight imperfections in a handcrafted piece are to be expected. Also, designs and colors may vary slightly.
